- Initially, applicants need to mention their Name, Father name, or Husband name and R/O specify as under.
- Age and Date of birth: The birth certificate which is issued by the district office and the birth certificate from the school which is recognized by the authorized educational institutions.
- Marks’ sheet of 10th class if it contains the date of birth.
- Any license (Pan card or Aadhar card or Driving license).
- Applicant Gender: Mention the gender field as Male or Female.
- Place of birth: Mention village, district, state.
- Relation name: Specify the relationship whether you are a married woman then you need to mention your husband’s name, if you are unmarried then you need to mention your father’s name.
- Here you need to declare the electoral roll of the following constituency as your name doesn’t have registered across the country.
- Location of Ordinary Residence:
- Fill up the complete postal address which includes the PIN code where you are residing ordinary and also attach the residence proof.
- Bank passbook or Ration card or Passport or Driving license or Electricity bill or Gas connection of the applicant or partners etc.
- In a final statement, you need to clearly declare that you have not been issued by EPIC at least once in your life till now in any constituency your name, and family members i.e; Father / Mother / Brother / Sister / Spouse with the same address.

- Particular day – Date and location.
- Applicant signature that you are declaring accurate information. and following the note that if you are doing any false declaration then it leads to illegal allegiance punishable under act section 31 which is people act, 1951 of panel code India.

Some of the Guidelines to fill up Age Declaration Form:
